Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi arrived at the facility unannounced to conduct a required annual visit. At 9:10 a.m., the LPA met with staff and explained the reason for the visit. At 10:12 a.m., Licensee/ Administrator Floro Cortes Jr. arrived at the facility.
RECORD REVIEW: Between 9:30 a.m. and 1:20 p.m., the LPA conducted a file review for ten (10) clients and five (5) staff and observed the following: Staff did not have current first aid. Client records were reviewed for but not limited to current Individual Program Plan (IPP), medical records, admissions agreement, and consent forms. The following was cited: One (1) out of ten (10) clients did not have a physician’s report/ medical assessments on file. Ten (10) out of ten (10) clients require updated appraisals/needs and service plan. Administrator’s Certificate is valid until 11/05/2026.
Due to time constraints, the LPA will return at a later date to complete the annual.
Pursuant to Title 22 Division 6 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 9099-D).
Exit interview conducted. The Administrator authorized staff Romeo Sapida to sign the report. A copy of the report and appeal rights were provided.
