Facility Evaluation Report
At 8:02am on 01/24/2023, Licensing Program Analyst (LPA) Jeffries arrived at the facility unannounced to conduct the annul, infection control inspection. LPA was not screed upon entrance to the facility and it was not mark and unclear where the single entry screening area was located. LPA met with facility staff Marcial San Juan (S1). LPA announced who he was and the reason for this visit being an annual, infection control inspection. S1 called Licensee Floro Cortes by cell phone and LPA spoke with Licensee and received verbal permission to conduct infection control inspection with S1.
At 8:15 am S1 and LPA conducted a cursory tour of the facility. This facility in located in an industrial area in the town of Saticoy. There are drive way gates located on three sides of the property, all gates are locked with the exception of the main entrance pedestrian gate. There are five buildings and a storage shed on the property, there are several areas on the outside of the buildings with in the fence for the clients to be outside with shaded areas through out the property. LPA observed the storage room where PPE and facility food storage are located. LPA observed at least seven days of non-perishable and a two days of perishable foods on hand. LPA observed six fire extinguishers located through out the facility property all currently charged in the green operating indicators. LPA observed four of the thirteen client rooms which are all double occupancy rooms. In general, the facility is clean, however residents rooms are cluttered but all exits are free and clear of obstructions as observed by LPA. LPA noted to the Licensee of broken refrigerators stored on the outside of the storage shed to be disposed of within seven days of this visit to ensure the safety of clients in care. LPA noted that there are no violation or citations issued as a result of the cursory tour of the facility.
At 10:22am Licensee and LPA conducted the infection control module portion of the annual inspection. LPA issued a technical for the unidentified single entry point and technical for the lack of screening but before the inspection ended the Licensee rectified both technical with a single entry point screening station and staff education. No citations or violations issued on this annual, infection control inspection.
