for it. Fresh fruit is being given but not daily. LPA did not observe fruits on the facility menu for the week of 1/10/22 to 1/16/22 when reviewed at 2:21 pm. During facility tour LPA observed a sufficient supply of fresh fruit. Interview with Administrator on 1/11/22 starting at 2:44 pm revealed that they do buy fresh fruit but it is not a preferred choice of the clients. Administrator stated that they are not offering clients fruit however, if they ask for it they will give them fruit. Concerns were that clients are not being given snacks. Interviews on 1/11/22 with random residents and staff starting at 1:07 pm revealed that everyday around 6 pm clients are given snacks. LPA observed snack schedule for December 2021 at 2:23 pm which indicates snacks are being given once daily. Concerns were that residents are not being offered activities. Interviews on 1/11/22 with random residents and staff starting at 1:07 pm revealed that activities are not being offered to residents. Interview with Administrator on 1/11/22 starting at 2:44 pm revealed that they do not offer any planned activities. During facility tour LPA observed card games, board games, coloring books, crayons, and crossword puzzles. Based on the information obtained during the course of the investigation the allegations are deemed substantiated at this time.
