Facility Evaluation Report
Licensing Program Analyst (LPA) Esther Cortez conducted a case management - deficiencies visit in conjunction with a complaint visit (Complaint control # 29-AS-20250620134439 ). The purpose of this visit is to issue citations for deficiencies observed during the complaint investigation which were not related to the complaint. Upon arrival LPA met with staff and explained the reason for the visit. Administrator Didi Daniels arrived shortly thereafter.
Starting at 10:10 a.m. the LPA reviewed records for Client 1 (C1) and observed they did not have a completed medical assessment on file even though being admitted to the facility since June 3, 2025. Per the Administrator, C1 has a medical appointment scheduled for next month and will obtain their medical assessment then.
Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D): Exit interview was conducted. Report and appeal rights were reviewed and provided to the Administrator.
