09/13/2023 and conducted by Evaluator Teresa Camara
COMPLAINT CONTROL NUMBER: 29-AS-20230913082131
Licensing Program Analyst (LPA) Teresa Camara conducted an initial complaint investigation visit. LPA met with co-administrator Amalia Soratorio and explained the reason for the visit.
LPA reviewed the staff schedule and staff training records starting at 1:09 p.m. All staff on schedule have criminal background clearances and are associated with this facility. LPA spoke with staff 1 (S1) at 1:39 p.m. and staff 2 (S2) at 1:42 p.m. Both S1 and S2 confirmed there are no other staff who work at the facility other than those on the staff schedule. LPA reviewed the training records and found the training for all staff was sufficient.
No deficiencies were observed. Exit interview conducted and report issued.
