The inspector’s account

Licensing Program Analyst (LPA) JoAnn Rosales conducted a Case Management - Deficiencies visit at the facility. LPA met with Administrator Michael O'Neill.

During facility tour on 11/22/2021 starting at 11:15 am with the Administrator LPA observed scissors in the Maricopa building Salon accessible to residents. Interview with staff # 1 (S1) on 11/22/21 at 2:23 pm revealed that they do not have a medical assessment on file for resident #1 (R1).

Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D).

Civil penalties issued in the amount of $250.00.

Exit interview was conducted, today's reports, civil penalties and appeal rights were reviewed and emailed to the Administrator.
