Facility Evaluation Report

Licensing Program Analysts (LPA) Esther Cortez conducted an unannounced Case Management -Deficiencies visit. The purpose of the visit is to issue citations for deficiency observed during the investigation of complaint control number 29-AS-20250820161847 not related to the complaint. LPA met with Community Care Director Iris Perez and Business Office Manager Melanie Lopez for Care Field and explained the reason for the visit.

During today's visit the LPA observed a video monitor in resident room #304 in the Matilija Building. Memory Care Director stated that the resident's family wanted to place the monitor in the room and they are turned away when the staff provides care.

The following deficiencies were observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiencies may result in civil penalties.

Exit interview was conducted. A copy of the report and appeal rights were provided.
