Facility Evaluation Report
Licensing Program Analyst (LPA) Teresa Camara conducted a case management - incident visit regarding an incident which took place on 3/25/2025.
LPA conducted interviews starting at 10:30 a.m. with the executive director (ED) David Scarlett and the resident care director (RCD) starting at 11:24 a.m. LPA conducted a brief tour of the Topa Topa memory care at 11:45 a.m.
Resident 1 (R1) was seated at a table in the dining area while LPA was touring. R1 was a bit confused but quite pleasant and happy to see the RCD. The incident involved R1 and two staff. Staff 1 (S1) was helping R1 with eating in the dining room on 3/25/2025 while staff 2 (S2) who was assisting another resident in their room. S2 overheard R1 say "owe" and reported to the RCD that S1 may have slapped R1 based on R1 stating "owe" but S2 did not see anything occur between S1 and R1. R1 tends to resist using utensils to eat and that was the likely reason R1 stated "owe".
Based on this information, it does not appear there were any personal rights violated. No deficiencies observed. Exit interview conducted and report issued.
