Facility Evaluation Report
Licensing Program Analysts (LPAs) Kelly Dulek and Valeria Conway conducted visit at this facility today to address concerns with the control of property. LPAs met with licensee representative Narinder Kumar at 02:49PM. Entrance interview conducted.
On 05/08/2026, licensee representative notified LPA Dulek and Licensing Program Manager (LPM) Desaree Perera via telephone that the current lease agreement between the licensee and the property owner ends on 05/10/2026 and the licensee did not plan to renew or extend the lease. LPA and LPM had a conversation with the licensee representative related to maintaining control of property and education related to unlicensed operation.
During today’s visit, LPAs confirmed with both the property owner and the licensee representative that there is no current lease agreement in place as of 05/11/2026, however licensee representative has continued operations at the facility, and therefore the facility is operating unlicensed.
Licensee stated they will notify all families via email that a new operator would be taking over operations effective on today’s date. Licensee representative had a meeting with all employees and issued final payroll checks to the staff.
A Notice of Operation in Violation of Law (NOVL) was issued. The licensee representative was advised that a retroactive civil penalty of $100 per day per tenant shall be assessed on the 16th day from the original date of the notice of Operation in Violation of the Law (May 15, 2026) for the operation of an unlicensed facility.
