Facility Evaluation Report
Licensing Program Analysts (LPAs) Martha Arroyo and Valeria Conway conducted a Case Management - Deficiencies visit in conjunction with a complaint visit (Complaint Control # 2 29-AS-20250513145158). The purpose of the visit is to issue citations for deficiencies observed during the investigation.
During the course of the investigation, it was discovered that staff had been pre-filling residents’ medications several days in advance. During interviews, employees admitted to preparing medications in separate containers at least two (2) days ahead of time and signing off on their administration in advance. Although the administrator stated that they were unsure why staff were pre-filling and documenting early and acknowledged that this practice was a clear violation of policy, staff have continued to engage in pre-filling the residents’ medications.
Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiencies are cited (refer to LIC9099-D).
