Facility Evaluation Report
On 3/20/2026 LPA Grace Donato made an unannounced Case Management-Incident visit. LPA met with Program Supervisor, Alona Poblete. LPA explained the purpose of the visit.
The visit is regarding the incident reported last February 19, 2026 to LPA, and followed by a written incident report on February 20, 2026.
Facility was hit by a SamTrans bus on February 18, 2026 around 10:00pm. No clients or staff were in the building when this event occurred.
LPA toured the facility and observed one corner of the facility boarded up. Facility is awaiting feed back from building owner. No timeline has been established. As of the moment, there is limited amount of clients who are able to go to the facility.
Facility has established a schedule where some clients come to the facility and some do zoom activities from the clients home or home facility.
Program Supervisor will keep updating LPA.
