Facility Evaluation Report
On March 26, 2026, Licensing Program Analyst(LPA) Murial Han arrived at the facility to follow up on a Decision and Order(exclusion) of staff #1 (S1). LPA met with co-administrator and explained the purpose of the visit.
During today's visit, the co-administrator provided a tour of the facility. LPA observed 2 semi-private and 2 private rooms, dining room, kitchen, living room, backyard and garage. There were no residents present as they were attending the day program.
According to the co-administrator, S1 is no longer working at the facility and S1 has been disassociated from the facility.
This report is reviewed and discussed with the co-administrator.
