Facility Evaluation Report

On 9/16/2025, LPA Grace Donato conducted an unannounced case management-incident visit. LPA met with Administrator Ann Yalung and explained the purpose of the visit.

This visit is regarding a report received by CCLD about the administrator hiring family members with no employment clearance to work at this facility and falsifying documentation to create paychecks for these individuals.

LPA reviewed the current LIC 500 and schedule of facility staff for the month of September and cross checked it the current Guardian Employee Roster. All staff currently working in the facility and are in the schedule has employment clearance and completed W4s. LPA also reviewed 3 staff files that were currently hired. Everything is complete and all updated.

LPA also observed a fingerprint scanner for the time in and time out of facility staff for payroll at the same time there's also a time sheet filled in by staff.

No deficiencies cited today. Report is reviewed and copy is provided.
