Facility Evaluation Report

On 6/4/2025, Licensing Program Analyst (LPA) Dominic Tobola conducted an unannounced annual inspection and was greeted by Administrator, Ann Yalung. The purpose of the visit is to continue the facility annual inspection.

The facility provides care for 5 clients, all of which were out of the facility attending day program at the time of visit. LPA conducted a review of facility staff records and found that records including health screening reports, first aid & CPR certification and annual training are completed. LPA conducted a review of client records and found that records including admissions agreements, identification forms, physician's reports, and service plans are completed.

The facility conducts fire safety and other emergency drills on bi-annual basis with documentation on file. LPA toured the facility and confirmed previous corrections needed had been resolved. LPA discussed the updating of facility administrator and will be forwarding the list of required items for the change of administrator process.

Annual inspection has been completed and report was reviewed with Administrator.

No deficiencies cited during today's visit.
