On 4/11/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct an unannounced Case Management visit to deliver an immediate exclusionary letter for S1. LPA was greeted by Charito Amoranto, Assistant Executive Director and explained the purpose of the visit.
An exit interview was conducted. A copy of the report along with the immediate exclusionary letter was left at the facility.
