The inspector’s account

On October 31, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met with Lead Staff, Gracia Santos and explained the purpose of today's visit.

LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility. The lead staff confirmed that this staff no longer works at the facility.

The exclusion letter was provided to the lead staff.
