On 6/24/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit to deliver an Amended report for a visit on 6/18/2025. LPA Calandra was greeted by Zach Pilkerton, Administrator and explained the purpose of the visit.
During today's visit, LPA Calandra reviewed the amended report with the Administrator and provided a copy of the report.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of this report was reviewed and left with facility representative.
