The inspector’s account

On April 7, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced annual visit. LPA met with Program Director, Nelson Gomintong and explained the purpose of the visit.

LPA toured facility and grounds. No accessible bodies of water or fire safety hazards observed. There is one communal area where clients participate in activities, one sensory room and one supply room. There are currently only 7 clients that attend the day program. There were two clients present during the visit, according to the program director, the other 5 clients were on an outing.

Rooms were observed to be clean, clear from tripping hazards, and spacious. LPA observed two restrooms at the facility that were clean and odor-free, equipped with paper-towels and liquid soap. Hot water temperature throughout the facility measured between 111-113 degrees F. Chemicals and sharps were locked and inaccessible to clients in care. According to Program Director, there are no clients taking medication at the program. LPA observed kitchenette, however facility does not prepare food for clients, as clients bring their food from their homes at the facility. Facility provides snacks and water to clients if requested. A comfortable temperature is maintained throughout the facility and lighting is sufficient for comfort. Fire extinguisher which were charged and inspected on July 2025. First aid kit was observed to be present. Carbon monoxide detectors and smoke alarms were observed to be in working condition.

LPA reviewed 5 client files and 5 staff files. Client records are updated, complete and signed. Staff records are complete, with training logs that have met the basic requirement. Emergency drills are being conducted and logged once a month. No citations are issued during this visit. Report is reviewed with Program Director and a copy is provided
