Facility Evaluation Report

On April 3, 2025, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced annual inspection. LPA met with Administrator, Nelson Gomintong and explained the purpose of the visit.

LPA toured facility and grounds. No accessible bodies of water or fire safety hazards observed. There is only one communal area where clients participate in activities, one sensory room and one supply room. There are currently only 4 clients that attend the day program. No clients were present during the visit as they were all out on a community outing. Room were observed to be clean, clear from tripping hazards, and spacious. LPA observed two restrooms at the facility that were clean and odor-free, equipped with paper-towels and liquid soap.

Hot water temperature throughout the facility measured between 105-118 degrees F. Chemicals and sharps were locked and inaccessible to clients in care. According to Program Director, there are no clients taking medication at the program. LPA observed kitchenette, however facility does not prepare food for clients at the facility. Facility provides snacks and water to clients as clients bring their own lunches from their homes.

A comfortable temperature is maintained throughout the facility and lighting is sufficient for comfort. Fire extinguisher which were charged and inspected on 09/2024. First aid kit was observed to be present. Carbon monoxide detectors and smoke alarms were observed to be in working condition.

LPA reviewed 4 client files and 4 staff files. Client records are updated, complete and signed. Staff records are complete, with training logs that have met the basic requirement.

No citations are issued during this visit. Report is reviewed with Program Director and a copy is provided
