Facility Evaluation Report

On April 14, 2026, Licensing Program Analyst (LPA) Murial Han arrived at the facility to deliver an Immediate Exclusion Letter and to obtain more information concerning to the incident that happened on March 12, 2026. LPA met with administrator and explained the purpose of the visit.

During today's visit, the administrator provided a tour of the facility. LPA observed some staff members were engaging in activities with residents and other were assisting residents with their lunches.

According to the administrator, S1 is no longer working at the facility and S1 has been disassociated from the facility.

During today's visit, LPA also interviewed 5 staff members regarding to the incident that happened on March 12, 2026.

This report is reviewed and discussed with the administrator.
