Facility Evaluation Report
On August 12, 2025, during the annual inspection, the administrator reported that resident #1 (R1) and resident #2 (R2) were watching TV and suddenly R1 ran towards R2 and bit R2's left arm causing a red mark.
The administrator reported that he and another staff member were present during the incident. R1's action was very fast and they immediately separated both residents and after the incident, they provided one to one supervision for both residents.
The administrator reported that R2 is doing well and they have reported the incident to both responsible parties, the facility of R1, GGRC and CCL.
During the visit, LPA observed both residents were in separate rooms and being supervised by a staff member. LPA observed both of them to be calm. R1 was doing art work and R2 was in a bigger room with an ice pack on the injury site.
The administrator stated that yesterday was R1's first day at the center and the facility will continue to monitor R1's behavior to ensure resident's safety and will communicate to R1's Social Worker to ensure the program is suitable for R1.
No deficient is cited today. This report is reviewed and discussed with the administrator.
