Facility Evaluation Report
Licensing Program Analysts (LPAs) Tiffany Holmes and Janet Ngallo conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPAs were welcomed by, identified themselves to, and discussed the purpose of the visit with Tracy Mcknight, Assistant Administrator.
According to the facility’s license, the facility has a maximum capacity of sixty (60) clients, all of whom must be ambulatory. During today’s inspection, there were a total of forty-five (45) clients in care.
LPAs, accompanied by staff, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Doors, windows, screens, and toilets were in working order. Personal Protective Equipment was present. The facility had sufficient space and equipment to facilitate meetings and client activities. Hot water temperature at taps accessible to clients were all compliant: Bathroom sink was 120 F.
There were no sharp objects, toxic chemicals/poisons, or open-faced heaters accessible to clients.
