The inspector’s account

Licensing Program Analyst, David Roman (LPA D. Roman) conducted an unannounced case management visit due to an incident that occurred on 02/03/26. LPA D. Roman is to conduct a tour of the facility to check on the health and safety of residents in care. LPA D. Roman gained access to the facility, identified himself, and met with Program Manager, Valerie Davis to discuss the purpose of today's visit.

LPA D. Roman's visit consisted of briefly interacting with residents and touring the eight homes of the facility. When touring the facility, LPA D. Roman observed residents engaging in the Life Enrichment Program. Overall, the facility did not have any immediate health and/or safety violations, outside construction was observed in the Caridad and Paz home. LPA observed areas to be blocked off for residnet safety by utilizing caution tape. LPA did not identify any residents with complaints about their health. LPA D. Roman toured the kitchen and pantry areas of all 8 homes to verify sufficient food for residents in care. LPA D. Roman met with Senior Resident Support Manager, Tina Quinagon, and reviewed the resident file for the incident pertinent to the resident in question. Residents file is complete, admission agreement, medical records, and individual program plan was reviewed.

An exit interview was conducted with Program Manager, Valerie Davis and Support Manager, Tina Quinagon to which a copy of this report along with Licensee/Appeal Rights was provided to them whose signature below confirms receipt of this report and their rights.
