Facility Evaluation Report
Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Administrator David Smith.
Today's visit was in response to an SOC341 Report of Suspected Dependent Adult/Elder Abuse, which licensee self-submitted to the CCLD San Diego Regional Office (received on 04/06/2023). According to the SOC341: in early April 2023, Licensee became aware of an incident involving Client #1 (C1), Client #2 (C2), Client #3 (C3), and Staff #1 (S1), Staff #2 (S2), Staff #3 (S3), Staff #4 (S4), Staff #5 (S5), Staff #6 (S6), and Staff #7 (S7). During today’s visit, LPA performed a facility tour and welfare check, finding that C1, C2, and C3 were safe. LPA also reviewed pertinent records and interviewed relevant clients and staff.
No deficiencies were cited for the above incident. Also, no deficiencies were observed during today's visit.
An exit interview was conducted with Smith, to whom a copy of this report, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
