Facility Evaluation Report

Licensing Program Analyst (LPA) Dawn Segura visited the facility to conduct an unannounced Case Management – Legal/Non-Compliance visit. The facility is currently under a non-compliance plan that includes increased monitoring and visits, and the purpose of the visit was to ensure ongoing compliance with regulations and laws and ensure the health and safety of clients in care. LPA was granted entry into the facility and met with Cherifer Manipon, Program Director, Cynthia Ruiz, Assistant Program Director, Jennifer Arzate, RN, Morgan Davis, Quality Assurance Manager, with whom she discussed the purpose of the visit.

During today’s visit, LPA toured the facility, observed clients in care, observed COVID-19 mitigation efforts and followed-up on the facility's adherence to Title 22 requirements, particularly personal rights, reporting requirements, emergency intervention, reporting requirements, and staff training.

No deficiencies were cited during today’s visit. An exit interview was conducted with Cherifer Manipon, and copies of this report and Licensee Rights (LIC 9058) were provided to the Program Director at the conclusion of the visit. The Program Director's signature on this report acknowledges receipt of copies of the report and rights.
