Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced annual required inspection. LPA identified herself to Direct Support Professional (DSP) Blount and was granted entry into the facility. Administrator Tafolla arrived shortly thereafter to assist with the inspection.
The facility is licensed for a capacity of four clients, of which three may be non-ambulatory, and is approved for delayed egress. At the time of the visit, all three clients were present in the home. The facility Administrator’s certification was reviewed and found to be current through December 2027.
LPA Correia, accompanied by DSP Blount and Administrator Tafolla, conducted a tour of of the facility. The ambient temperature inside the home measured 71 degrees Fahrenheit. Smoke alarms and the carbon monoxide detector were observed to be operable, and the fire extinguisher was present and up to date. Medications, toxic solutions, and sharps were secured in locked cabinets and drawers and were inaccessible to clients. Adequate supplies of linens, PPE, and a fully stocked first aid kit were observed. The last disaster drill was conducted on February 13, 2026.
