Facility Evaluation Report
Licensing Program Analyst (LPA) Correia conducted an unannounced visit to verify the facility’s closure following the Department’s revocation of the facility license. LPA identified herself and was granted entry by the former Administrator, Nigro, (S4) to whom the purpose of the visit was explained.
LPA toured the facility and confirmed that there are no clients in care. Additionally, LPA interviewed S4 to confirm Staff (S1- S3) are not currently working at a facility or Home Care Organization (HCO), and reviewed the conditions and requirements set forth by the Department regarding the facility closure, including a lifetime exclusion for former S1 and two-year exclusions for S2 and S3 from working in a licensed facility or HCO. During today’s visit, LPA retrieved the facility’s license in accordance with the closure process.
On January 21, 2026, the Department conducted a Health and Safety visit to the clients’ new location and confirmed their placement and well-being.
An exit interview was conducted with former Administrator Nigro. A copy of this report was provided, and the signature below confirms receipt of the document.
