The inspector’s account

Regional Manager (RM), Jerry Romero, Licensing Program Managers (LPM), Robyn Clark and Simon Jacob, and Licensing Program Analyst (LPA), Debbie Correia conducted an unannounced Case Management - Legal/Non-compliance visit. RM, LPM’s, and LPA were greeted and allowed entry into the facility by Administrator Nigro, to whom they explained the purpose of the visit.

On 1/16/2026, the licensee was issued a Temporary Suspension Order and clients were relocated. Today, RM, LPM’s, and LPA toured the facility and verified there were no clients in care.

During the visit a facility tour revealed hazardous toxins were accessible to clients in care. LPM Jacob observed three (3) cans of paint that were not locked or secured. In addition, the San Diego Regional Center (SDRC) Staff (OS1, OS2) who were present during the visit reported medication errors, which included pre-poured medications for three (3) out of five (5) clients in care, as well as PRN medications that were not labeled per mandate, which was also observed by RM Romero. Based on interviews, facility staff (S1) indicated that the PRN medication was "communal" and used as needed by all five (5) clients.
