Facility Evaluation Report

Licensing Program Analyst (LPA) Debbie Correia arrived on 1/16/2026 for an unannounced inspection to follow up on an incident report submitted by the facility on 1/13/2026. LPA Correia identified herself and explained the purpose of the visit to Licensee Linda Rucker , who granted entry.

On 01/16/2026, the Licensee was issued a Temporary Suspension Order alleging that the facility failed to ensure that a client (C1) was adequately supervised and as a result, the Licensee (S1) engaged in inappropriate sexual contact with C1.

The Department has concluded an analysis and has determined that a civil penalty is warranted for physical abuse. The Welfare and Institutions Code Section 15610.63 for physical abuse means any of the following: Oral copulation, as defined in Section 287 or former Section 288a of the Penal Code, Sexual penetration, as defined in Section 289 of the Penal Code, Lewd or lascivious acts, as defined in Section 288 of the Penal Code. This is evidenced by the fact that S1 sexually abused C1, who was incapable of giving consent.

Today, 1/16/2026 , the Department will be issuing a civil penalty per Health and Safety Code Section 1548(f)(1)(A) for a violation that the Department constitutes as physical abuse in the amount of $10,000.

Exit interview conducted. A copy of the report issued. Appeal rights provided to Licensee Linda Rucker and signature on this report acknowledges receipt of the appeal rights, found on page two of LIC 421D.
