Facility Evaluation Report
Licensing Program Analyst (LPA) Correia conducted an unannounced Case Management visit to perform a health and safety check for clients in care. LPA was greeted by Licensee Knight, identified herself, was granted entrance, and met with Caregiver Nigro, to whom the purpose of the visit was explained.
The facility is licensed to serve a capacity of six (6) clients, and the current census is five (5). One (1) client was present during today’s visit.
During the visit, LPA conducted a facility tour, spoke with staff, and briefly interacted with Client 1 (C1). The facility tour revealed no immediate health or safety concerns. The facility was observed to be sanitary and in good repair, and C1 appeared well-kempt and .engaged in an activity at the time of the visit.
No deficiencies were cited during today’s visit.
An exit interview was conducted with Caregiver Nigro at the conclusion of the visit. A copy of this report and the Licensee/Appeal Rights (LIC 9058 3/22) were provided. The signature below confirms receipt of these documents.
