Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Correia conducted an announced Pre-licensing Inspection and Comp III visit. LPA met with Applicant Christy Alvarez, identified herself, was granted entry into the facility, and stated the purpose of the inspection. The facility’s Fire Clearance was granted on September 22, 2022 for four (4) developmentally disabled clients, ages 18-59, all of whom must be ambulatory.
LPA Correia, accompanied by Applicant Alvarez, conducted a during a facility inside and out. LPA Observed one or more operating smoke alarms and carbon monoxide detectors that meet statutory standards. The facility has no bodies of water on the property. Per applicant Alvarez there will be no ammunition kept on facility grounds. Clients have a sufficient supply of clean linen in good repair. Client bathrooms were equipped with a toilet, sink, and shower in operating and sanitary conditions. Facility is equipped with a locked storage cabinets for medications, toxins, and other dangerous supplies (e.g. knives). The facility also has an activities schedule/list planned for clients in care. LPA also observed a common room, and a shaded outdoor area for clients to congregate and/or participate in activities.
In accordance with the Department’s Infection Control, LPA provided technical assistance, evaluated, and observed the facility's plan for implementation of their infection control plan including but not limited to disinfection, testing surveillance, and screening protocols for universal entrance, as well as the use of personal protective equipment.
Based on today’s inspection the facility is not ready for Licensure due being over capacity. Upon relocation of one (1) client in care LPA Correia will conduct a follow up visit to ensure capacity compliance . An exit interview was conducted and a copy of this report and Licensee/Appeal Rights (9058 01/16) will be provided to Applicant Alvarez, whose signature on this form confirms receipt of these documents.
