02/18/2025 and conducted by Evaluator Tiffany Holmes

Facility staff are not ensuring that trash is removed from the property.

Licensing Program Analyst (LPA)Tiffany Holmes contacted the facility to deliver findings for a complaint investigation via tele-virtual. LPA identified herself to, and explained the purpose of the visit and the basic elements of the complaint with Domingo Cuison, Director.

LPA Barfield conducted the initial visit on February 26, 2025 and conducted a tour of the facility.

LPA Holmes has conducted interviews with staff, outside sources and obtained and reviewed pertinent records. On February 18, 2025, a complaint was received regarding facility staff are not ensuring that trash is removed from the property. Interviews revealed that there wasn't any trash outside the facility. Interviews revealed that the facility administrator was getting rid of old furniture, old Protective Personal Equipment (PPE), expired sanitizer and boxes. Interviews revealed that they had called a junk removal agency that came out a couple of days later and removed everything the administrator sat outside for removal. Interviews with staff revealed there wasn't any trash in the walkways and that the parking lot was clear and safe for the clients. Other interviews revealed that the facility is neat and clean and does not have trash laying around. Interviews with an outside source revealed that they observed the trash/junk outside of the facility and that they called the administrator and told them they needed to clean everything up. Interviews with an outside source also revealed that they saw the trash/junk outside around 8am or so and that the facility staff had it cleaned by 2pm that same day that the outside source observed it.

The investigation did not produce supporting evidence or supporting witness statements to substantiate the facility staff are not ensuring that trash is removed from the property. Based on the evidence obtained from interviews, the complaint allegation is unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted with Domingo C, Staff and a copy of this report along with Licensee/Appeal Rights (LIC 9058 03/22) was provided at the conclusion of the visit.
