Complaint Investigation Report
Allegation #1: Staff handled clients in a rough manner.
The complaint alleged that a staff member slammed a client against a wall while the client was exhibiting behavior. On June 1, 2026, the department interviewed the Administrator (A1), who denied the allegation and stated that staff members are trained not to touch clients unless they are helping them move around and never to touch them when they are displaying challenging behaviors. Instead, staff members are instructed to redirect clients and help calm them down.
During the same investigation, the department interviewed four staff members (S1-S4), all of whom denied ever handling clients in a rough manner. They also emphasized their commitment to respecting clients' rights. Additionally, the department interviewed four clients (C2-C5), who reported that staff members did not treat them roughly and mentioned that staff play with them every day. On June 01, 2026, the department interviewed the Administrator (A1) of the facility where the client C1 was living at the time. A1 denied being aware of any allegation against staff who had roughly handled C1 at the day program.
On June 1, the department observed some clients playing outside and noted that, while some were exhibiting behaviors, staff members successfully redirected them and helped calm them down.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is unsubstantiated.
