Complaint Investigation Report
Allegation #1: Staff pushed residents in care.
The complaint alleged that staff at the day program pushed Client 1 (C1). On June 1, 2026, the department interviewed the Administrator (A1), who denied the allegation. A1 stated that on April 20, 2024, around 10:30 a.m., C1 started exhibiting behaviors. The staff moved to avoid getting hit, and A1 attempted to redirect and calm C1. A1 also noted that staff are trained not to touch clients physically.
Additionally, the department interviewed four staff members (S1-S4), all of whom denied the allegation. They stated that no staff member at the facility has ever pushed a client for any reason and that they have received training on clients' personal rights. If a client exhibits challenging behavior, staff are trained to redirect the client by guiding them without physical contact.
The department interviewed four other clients (C2-C5), who confirmed that staff do not push or hit them. On June 1, 2026, the department observed clients at the facility exhibiting a range of behaviors. Staff remained compassionate and calm, helping clients calm down. On June 2, 2026, the department interviewed the Administrator (A1) of the facility where client C1 lives. A1 stated that the client had reported the incidents to them and that they had sent an unusual Incident Report to Community Care Licensing, the Ombudsman, and the Placement agency. The department also reviewed a facility note dated December 20, 2024, which detailed the incident that occurred that day.
