Facility Evaluation Report

On 01/22/2026, Licensing Program Analyst (LPA) Janette Romero visited the facility in response to an incident report submitted by Administrator Jenny Castellanos reporting on 01/18/2026 Resident 1 (R1) observed Staff 1 (S1) slap Resident 2 (R2) on the left side of their head during lunchtime. LPA was greeted and granted entry by Activity Director (AD) Fely Dizon who was informed of the purpose of the visit. Administrator Castellanos arrived to the facility to meet with LPA.

During today's visit, LPA toured the facility with AD Dizon, conducted interviews, and obtained copies of pertinent records. Administrator Castellanos was interviewed in regard to the incident on 01/18/2026. However, during the course of the investigation Administrator Castellanos reported she is in the process of submitting an additional incident report to Community Care Licensing due to receiving allegations that Staff 2 (S2) and Staff (3) verbally abused multiple residents in the facility. At this time, the incident dates/times are unknown. Administrator Castellanos reported S2 and S3 have been relieved from their duties in the facility pending an internal investigation into the allegations. During the visit, LPA did not observe any immediate health or safety concerns nor did LPA observe S2 or S3 present in the facility. No deficiencies were cited during today's visit. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator Castellanos.
