The inspector’s account

On April 27, 2026, Licensing Program Analyst (LPA), Kyle Wellington, made an unannounced visit for the purpose of the facility's closure and met with Administrator (Admin), Rolando Corpuz.

On March 30, 2026, LPA received a letter from the Admin and Licensee, Leslie Wang, requesting closure of the facility. On April 7, 2026, LPA conducted a case management-health and safety check at the facility. On April 20, 2026, LPA received a Resident Relocation Report from the Admin and Licensee. Also on April 20, 2026, Regional Office (RO) and LPA received a Closure Report from the Admin and Licensee that included a scanned copy of the Facility License. LPA requested the original Facility License from the Admin. Admin stated he would look for the original and ask the Licensee if she has it then send it to the RO if found. The facility has a photocopy of the Facility License on the wall.

The Licensee initiated the facility closure on March 30, 2026 with an effective closure date on April 30, 2026.

During today's visit LPA and Admin inspected the inside of the facility and found that there were no residents. Admin reported the that all 5 residents had been previously relocated. LPA and RO had obtained prior to today’s visit the locations of where each resident is currently residing.

LPA explained to the Admin that the license is no longer valid, and therefore no required care and supervision should be provided in the home unless the state approves licensure in the future.

An exit interview was conducted with the Administrator, and a copy of this report was provided to the Administrator.
