Facility Evaluation Report
On 7/28/2025, Licensing Program Analyst (LPA) Valerie Flores arrived at the facility to conduct an unannounced required 1-year inspection. Upon arrival LPA was greeted by House Manager, Efren Cuaresma, who granted LPA entry into the facility. LPA explained the purpose of visit and conducted a tour of the facility alongside Efren. During the time of visit, four (4) out of four (4) clients were attending day-program and would not return until 3PM. LPA observed the following during today's visit:
The physical plant is a single-story structure. Clients bedrooms were equipped with the required bedding, furniture, and functional lighting. Additional linen and towels are available for clients and appear to be in good repair. The facility met the (2) two-day supply of perishable food and (7) seven-day supply of non-perishable items. LPA observed emergency food and water supply located in the garage. An additional fully stocked freezer was located in the garage. Dishes and utensils were in sufficient in supply and in good repair. Indoor/outdoor passageways were free of obstructions. LPA observed a fully charged fire extinguisher in the dining room. Smoke and carbon monoxide detectors were tested and were observed to be fully operable. A separate locked cabinet was located in the kitchen for centrally stored medication. Water temperature measured at 105.8-degrees Fahrenheit meeting within the department's requirement. There is a telephone working at this location. Per House Manager, Efren, there are no firearms or ammunition on the premises.
