The inspector’s account

On 10/29/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct 1 year required visit. LPA was greeted and granted entry by administrator Lupo Leyva, and met with administrator Bernadette Balungcas, where LPA explained the purpose of the visit. At the time of the visit there was (2) Staff and (0) clients present, as the clients were at the day program. LPA verified facility contact information with no changes needed, the facility annual fees were observed to be paid and the governing body was observed to be in good standing. Below are the observations made during today's inspection:

The facility was observed to be clean and clutter free with the passageways free from any obstructions. There are no pools or bodies of water or guns and ammunition on the premises. The facility was observed to have fully charged fire extinguishers. The last emergency disaster drill was conducted on 10/15/25. The hot water temperature was found to be within regulatory limits, measuring at 118.4 degrees Fahrenheit. The smoke and carbon monoxide detectors were tested and observed to be operable.

The medications, chemicals, sharps and other hazardous items were observed to be locked and inaccessible to clients in care. A records review was conducted of both staff and client files. All staff present were observed to have obtained criminal record clearance and associated to the facility. Staff possessed valid Cardio Pulmonary Resuscitation certification and to having valid Administrator certification for Bernadette Balungcas expires on 07/09/26. The client files were observed to have admission's agreement, medical assessment and Individual Program Plan (IPP). The Personal and Incidental (P&I) funds were reviewed with no discrepancies found. The facility was found to be in compliance with the California Code of Regulations (Title 22, Division 6, Chapter 6), as there were no citations issued during today's inspection.

An exit interview was conducted and a copy of this report, and confidential names list (LIC811), was reviewed provided to Administrator Berandette Balungcas.
