Facility Evaluation Report
On 10/31/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by Jaqueline Sanchez, Administrator, where LPA explained the purpose of the visit. At the time of the visit there was (1) staff and (0) clients present, as the clients were at the day program. The facility is licensed to serve (4) ambulatory adults ages 18 through 59.
LPA conducted a tour of the interior/exterior areas of the facility. The facility was observed to be clean and clutter free. The facility is a two-story structure consisting of three (3) client bedrooms, two (2) bathrooms, two (2) living rooms, kitchen, dining room, and garage/gym. There are no pools or bodies of water on the premises. The food supply was observed to be sufficient as there was a (2) day supply of perishable and (7) day supply of non-perishable food items. LPA observed the sharps to be in a locked safe, stored on top of the kitchen counter. The chemicals were observed to be inside the locked laundry room. The smoke alarms and carbon monoxide detectors were tested and found to be to be operable. The facility is conducting emergency disaster drills on a monthly basis, with the last drill being held on 10/01/25. The fire extinguishers were last services 09/20/25. The facility is equipped with emergency supplies flashlights, night lights and first aid kit and manual.
A records review was conducted of both client and staff files. Client files were observed to have medical assessments, Individual Program Plans (PP) and completed admission agreements. Client Personal and Incidental funds were reviewed with no discrepancies found. The staff files were observed to have valid CPR and Jaqueline Sanchez has a valid administrator certificate that expires 08/07/26. The facility annual licensing fees have been paid in addition, LPA verified contact information and will update accordingly. There was no deficiencies issued during today's visit.
An exit interview was conducted and a copy of this report was reviewed and provided to administrator Jaqueline Sanchez.
