The inspector’s account

On 04/06/2026, Licensing Program Analyst (LPA) Jacqueline Shaw-Ross, arrived at the facility to conduct a annual required visit. LPA was greeted and granted entry by Administrators Arni and Hamid Montazer where LPA explained the purpose of the visit. The facility is approved for (6) non ambulatory residents of which 1 may be bedridden. At the time of the visit there were (5) residents present of which one resident was asleep during LPAs visit.

The facility is licensed to serve age range 60 and over and an approved hospice waiver for (6) with (2) resident currently receiving hospice services. During today's visit LPA observed annual fees were paid up to date, and the governing body was observed to be active and in good standing.

The facility interior and exterior areas of the facility were observed to be clean, and free of any obstructions. The food supply was observed to be sufficient as the requirement was met. The facility has a clean pool that is housed behind a locked fence. There are no known guns or ammunition. The emergency disaster drills are being conducted on a quarterly basis with the last drill being on 04/01/2026. The fire extinguishers are fully charged and operable. The hot water measured to be 126 degrees Fahrenheit which is over the regulatory limit, however Administrator was able to adjust the temperature at the time of visit. Hot water retest measured at 118 which is within regulatory limits. The medications, sharps, chemicals and other hazardous items were observed to be locked and in accessible to residents in care. The facility is armed with a security alarm and video surveillance in common areas. Medications are being administered per physicians orders.

All staff present had the required training and possessed valid CPR certification. Staff have criminal record clearance and are associated to the facility. The Administrator Arni Montazer's administrator certification expires on 08/12/2026. Resident files had the required documents such as admissions agreement, medical assessment and appraisals. Based on today's inspection no citations were issued. An exit interview was conducted and a copy of this report was reviewed and provided to Arni Montazer, Administrator.
