Facility Evaluation Report

LPA Liliana Silveira conducted an unannounced visit to conduct a required Annual Inspection. LPA knocked at the front door to the facility multiple times and waited for a period of time. LPA attempted to contact the phone numbers that are on file; there was no answer for the facility line and the mobile line was disconnected. LPA waited to for a period of time and attempted to knock and call the facility line again, there was no answer.

LPA conducted research and was able to obtain a third phone number. LPA was able to speak to the Licensee Loretta Talia who informed LPA that the Director was currently at the hospital with a client and staff would be present at the facility later in the afternoon.
