Complaint Investigation Report

This information was verified with cleaning invoices and statements from management. LPA Strong additionally interviewed multiple residents who reported no issues. Lastly, Assistant Executive Director and Dining Manager state they are not aware of any resident who spits in public places.

Based on LPA's interviews, observations, and records reviews there is not a preponderance of evidence to prove alleged violation occurred, therefore the allegation is unsubstantiated. An exit interview was conducted with Executive Director Daniel Slaughter to whom a copy of this report, and the Licensee/Appeal Rights (LIC 9058 01/16) were provided.
