Licensing Program Analyst (LPA) Sabel Martinez conducted an unannounced Collateral visit. The LPA introduced himself and discussed the purpose of the visit with Director of Health Services Cha Cha Doles.
During the visit, the LPA conducted interviews with staff. No deficiencies were cited on today's date.
An exit interview was conducted with Doles, to whom a copy of this report, and the Licensee/Appeal Rights (LIC 9058), were provided.
