Home Care Organization Evaluation Report

Enforcement Analysts (EA), Jane Cong-Huyen and Mila Quinto, with the Home Care Services Branch (HCSB) conducted an inspection for the purpose of a closure visit on 8/19/2025. The HCO license expired on 5/31/25.

EAs walked the premises located at 36972 Wildwood Canyon Rd. Yucaipa, CA 92399 to verify any signs of operation. During the inspection, the EAs did not observe any visible business advertisements or signage for the HCO and verified there was no presence of an HCO operating at this time. EA Cong-Huyen called the phone number listed for this HCO. EA spoke to Jackie Huber and she stated that she was the owner/licensee for Chanan Group but since has retired and closed the HCO business beginning of March 2025.

Based upon information gathered, EAs confirm that this HCO is no longer in operation and is closed at this time.
