Facility Evaluation Report
On 11/21/2025 at 8:10 AM Licensing Program Analysts (LPA) La Vette Farlow arrived to conduct the required comprehensive annual inspection. LPA was greeted by Administrator, Rachel Padilla, and introduced self and stated purpose of the visit. Rachel informed LPA that she was leaving to take two clients to the doctors for a physical and she didn't have any staff available to complete the inspection.
LPA Farlow returned to the facility at 12:25 PM to complete the inspection a second time. LPA knocked on the door and rang the doorbell with no response. LPA called the telephone number listed on the profile and Rachel answered and inform LPA that she would be returning shortly and that they are finishing lunch. Rachel arrived to the facility at 12:48 PM. Rachel escorted LPA through the garage into the facility. LPA was informed that facility has 4 clients. Two clients are current at the day program and the other two clients returned with Rachel from the doctors visit.
The facility has 4 clients bedrooms, 2 bathrooms, extra bedroom for staff with full bathroom, office area, kitchen, dining area, family room, living room, laundry area, attached garage, and backyard. The facility is vendorized by Inland Regional Center. LPA completed a walk through of facility, review of records, medication and P&I audit.
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a temperature of 70 degrees Fahrenheit. LPA inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs and sufficient lighting. LPA inspected client bathrooms; bathrooms were clean and appliances were found functional. Water temperatures tested at 124.3, 102.7, and 128.2 degrees Fahrenheit. A Deficiency cited .
