Facility Evaluation Report

Licensing Program Analyst (LPA) Paola Guerrero conducted a case management visit based on deficiency observed during complaint investigation # 56-AS-20260429095229 . LPA met with Program Manager Danielle Edwards,and discussed the purpose of the visit.

During the collection and review of records LPA discovered that the Facility did not report incident associated to complaint # 56-AS-20260429095229 to Community Care Licensing. Program Manager informed LPA that Inland Regional Center was notified of the incident however, facility did not notify Community Care Licensing Department.

One deficiency will be issued for reporting requirements.

A exit interview was conducted and a copy this report along with a copy of the appeal rights were provided to Program Manager Danielle Edwards.
