Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Program Aide, Kenia Rodas and Case Manager, Kara Mitchell and discussed the purpose of the visit. The facility is an Adult Day Program (ADP) with a license capacity of (48) and a current census (17). LPA conducted an overall inspection, which included, but was not limited to, the following:
Operation/Physical Plant: The facility was observed to have sufficient care staff to assist clients at program. Indoor and outdoor passageways were kept free of obstruction. The facility has no outdoor bodies of water. Activity areas were maintained clean and odor free. The facility has sufficient activity supplies and operating vans for client transportation. Client bathrooms were maintained in a safe and sanitary condition. The hot water temperature tested at 114 degrees F. The facility was equipped with operating smoke/carbon monoxide alarms, fully charge fire extinguishers and telephone service. Cleaning supplies, sharps, and other dangerous items were kept locked. The facility's kitchen area was maintained clean and snacks are provided by the facility. The facility has a sufficient supply of bottled drinking water for clients.
Record Review: Staff records reviewed had First Aid/CPR certifications, criminal record clearances, job training, and health screenings. Client records reviewed had admissions agreements, needs and service plans, and medical assessments. The facility has an infection control plan and emergency/disaster plan for review.
Based on LPA observations, no deficiencies were cited during today’s visit. An exit interview was conducted, and this report (LIC809) was discussed and a copy provided to Case Manager Kara Mitchell at the conclusion of the visit.
