Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced case management visit. LPA was granted entry into the facility and met with Administrator, Chaztiney Cloud. The purpose of the visit is to follow-up on a Special Incident Report (SIR) submitted to Community Care Licensing (CCLD) by facility staff.

During today's visit, LPA obtained documentation related to incident that occurred on May 3, 2026 involving Client #1 (C1) . LPA observed no immediate health and safety concerns and all three (3) clients were present at the facility. The Administrator was informed that the incident requires further review by the Department.

An exit interview was conducted where this report LIC809 was discussed and copy provided to Administrator Cloud.
