Facility Evaluation Report

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit to the facility and was granted entry. The purpose of the visit is to follow-up on an incident report received by Community Care Licensing Division regional office on 02/21/2025. LPA Malcore met with Administrator, Chaztiney Cloud and discussed the purpose for the visit.

During today's visit, LPA conducted interviews with outside parties, the Administrator, clients, and staff regarding an incident which occurred at the facility on 02/16/2025 involving Client #1 (C1).

No deficiencies were cited pertaining to the above incident and no health and safety concerns were observed.

An exit interview was conducted where this report was discussed and a copy provided to Administrator Cloud at the conclusion of the visit.
