Complaint Investigation Report
The third allegation indicates Staff are making client sleep on the floor. Interviews with S1 and S2 reveal that at no time is a client made to sleep on the floor. S1 and S2 state that a client may lay to the ground but has never been told to sleep on the floor. C1, C3 stated to have never been made to sleep on the floor, nor witnessed any other client sleep on the floor.
Based on the information obtained, the allegations are UNSUBSTANTIATED. A finding of UNSUBSTANTIATED means that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur.
An exit interview was conducted, and a copy of this report was discussed and provided to DSP Victoria Chitgian.
